Application for revocation of electronic certificates dedicated to identity card owners

In case of loss, theft or compromise of my identity card and the disclosure of the associated PIN code, I request the revocation of certificates linked to my digital identity.

I recognise that the revocation is permanent.

This application for revocation is not a substitution to a theft report. I should contact the police to report the loss or theft.

This application for revocation has to be completed by a request for a renewal from the Mairie , in order to get new identity card and certficates.

Incomplete requests will not be processed. In that case, please go to the Mairie in person.
